Sarah from The Screen Savers shows us how to hack your Windows XP start button. For those who've tried this on Vista, does it work? Video after the jump.
Its even easier if, right before you save, go the task manager (ctrl+alt_del) and close explorer.exe and then save then go to the new task button and run explorer.exe ... ALL without Restarting.

Tried replacing/saving while explorer.exe was off with task manager. This method won't work. Windows will automatically replace explorer.exe the moment it is replaced with something else than the original.
So, you actually do have to restart in command prompt in order to replace the explorer.exe
Kinda sucks, but yeah.
